Terms of Service & Liability Waiver

Grip BJJ: Terms of Service & Liability Waiver

1. Liability Release & Assumption of Risk

By signing this agreement, I acknowledge that Brazilian Jiu-Jitsu, weightlifting, and group fitness involve strenuous physical activity and the risk of serious injury.

  • I voluntarily assume all risks associated with participation.

  • I certify that I am in good physical health and have no medical conditions that would prevent my safe participation.

  • I release Grip BJJ (and its subsidiaries, Lumina Yoga and Forge Performance Lab) from any liability for injuries, loss, or damage to personal property.

2. Membership Billing & Cancellations

  • Auto-Pay: I authorize the studio to charge my provided payment method on a recurring monthly basis.

  • Cancellation: I understand that cancellation requests must be submitted via the member portal or email at least 15 days prior to my next billing date.

  • Refunds: Membership fees, class packs, and workshop tickets are non-refundable and non-transferable.

3. Code of Conduct & Hygiene

To maintain a safe and respectful "tribe" environment, I agree to:

  • Maintain high standards of personal hygiene (clean Gi/uniform for every class).

  • Trim finger and toe nails to prevent injury to training partners.

  • Follow the instructions of the Professors and Coaches at all times.

  • Respect the "Tap Out" rule: Any physical or verbal signal to stop must be honored immediately.

4. Media Release

I grant Grip BJJ permission to use photographs or video footage taken during classes for promotional and social media purposes. (If I wish to opt-out, I must notify management in writing).

5. Privacy Policy

My personal data will be stored securely in the studio’s management software and will not be shared with third parties. I consent to receiving transactional emails regarding my bookings and account status.

How to use this in your Software Demo:

  • The Checkbox: Create a field where the user must check "I agree to the Terms & Conditions."

  • The Signature: If your software supports it, add a "Signature" box to show how a member can sign using their finger on a tablet or a mouse on a desktop.

  • The File Upload: Use this text to generate a dummy PDF that can be "attached" to a member's profile to show how the software stores documents.
